 Forms:

The form, PA-1000, is a pretty easy form to fill out, if you are a property owner, and fit in one of the categories, you will need to provide proof of your “paid” property taxes, both spring and fall for 2020, along with proof of your income.

If you are a renter, and fit in one of the categories, you will need the Rent Certificate filled out by your landlord (this is found in the PA Property Tax or Rent Rebate booklet), proving that you paid rent in 2020, along with proof of your income.

There are booklets here at 160 S. Second Avenue, Suite C, and we fill them out for our constituents.

If you have any other questions regarding this program, please do not hesitate to contact our office.
