Please use the document below to determine whether your student-athlete requires an impact test for the 2021 fall season. Keep in mind, these student-athletes are those who have signed up, via the fall sports sign up link on the Athletics home page, to participate in sports for the coming fall season and who will need an impact test for this school year. If your student-athlete's name is not on the document below, this means one of two things. Either your student-athlete has not signed up for sports or they do not require an impact test for the fall season. If you are unsure of whether your student-athlete requires an impact test, please email our Athletic Trainer, Mr. Kevin Kaufman, at kevin.kaufman@acvsd.org with your son or daughter's first and last name.
Please keep in mind that all medical forms are due by the first date of practice, August 16. Football players are required to have their paperwork submitted by the first date of heat acclimation, August 9. Student-athletes will not be permitted to participate in team practices until all medical paperwork has been collected and approved by the Athletic Trainer.
